Roles & Responsibilities

• Evaluate patients and create individualized, meaningful treatment plans that promote independence
• Deliver skilled occupational therapy interventions to help patients regain daily living skills
• Collaborate closely with nurses, PTs, SLPs, and other team members to coordinate care
• Document patient progress thoroughly and adjust plans based on clinical response
• Educate patients and caregivers on adaptive strategies, home safety, and therapeutic exercises
• Apply clinical reasoning to prioritize caseloads and ensure high-quality, patient-centered care

Qualifications

• Active California Occupational Therapy license or clear eligibility to obtain one
• Graduate of an accredited Occupational Therapy program (MS/OTD or equivalent)
• Strong clinical reasoning and documentation skills appropriate for SNF settings
• Excellent verbal and written communication skills for patient and team interactions
• Passion for patient-centered care and working in a multidisciplinary environment
• Previous SNF experience is a plus but motivated new grads with strong foundational skills are encouraged to apply

Education

• Graduate of an accredited Occupational Therapy program (MS/OTD or equivalent degree)

Knowledge & Skills

• Skilled in patient evaluation, ADL assessments, and treatment planning
• Familiarity with adaptive equipment and strategies to maximize independence
• Proficient with clinical documentation and progress note best practices
• Comfortable collaborating in a multidisciplinary team and communicating care plans
• Basic understanding of Medicare/Medicaid documentation and SNF workflows
• Strong patient education techniques and caregiver training abilities
