Roles & Responsibilities

• Provide skilled physical therapy evaluation and evidence-based interventions to SNF residents
• Create, implement, and update individualized treatment plans that promote function and mobility
• Document assessments, progress notes, and plans accurately and timely in the medical record
• Collaborate with interdisciplinary teams to coordinate care and achieve patient goals
• Educate patients and families on home programs, safety, and strategies to maintain gains
• Monitor outcomes, adjust interventions as needed, and ensure regulatory compliance

Qualifications

• Active California Physical Therapy license (required)
• Experience delivering skilled therapy interventions in a post-acute or SNF setting preferred
• Strong documentation skills with attention to clinical detail
• Comfortable managing caseloads and coordinating with nursing and rehab staff
• Excellent communication skills and a patient-centered approach
• Ability to work full-time for the 13-week contract period

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled assessment and treatment planning for older adults
• Gait training, transfers, balance and functional mobility interventions
• Understanding of SNF documentation requirements and regulatory standards
• Familiarity with common post-acute diagnoses and comorbidity management
• Proficiency with electronic medical records and timely charting
• Strong clinical reasoning and outcome tracking
• Team collaboration and discharge planning
• Safe manual handling and assistive device training
