Roles & Responsibilities

• Assess resident mobility and functional status, then design and implement targeted therapy plans
• Deliver therapeutic exercises and gait training to improve independence and quality of life
• Monitor resident progress, adapt interventions, and communicate outcomes to the interdisciplinary team
• Document care accurately and timely in compliance with SNF and regulatory requirements
• Educate residents and families on safe mobility strategies and home exercise programs
• Collaborate with nurses and other clinicians to ensure coordinated, person-centered care

Qualifications

• Active California PTA license required
• Experience in a Skilled Nursing Facility preferred
• Strong clinical judgment and excellent communication skills with residents and team members
• Comfortable with documentation and basic SNF regulatory expectations
• Able to work full-time for the 13-week contract period

Education

• Associate degree from an accredited Physical Therapist Assistant program or equivalent certification

Knowledge & Skills

• Hands-on experience with therapeutic exercise, mobility training, transfers, and assistive devices
• Skilled in resident assessment and writing/update of treatment notes
• Familiarity with SNF workflows and interdisciplinary team communication
• Good clinical reasoning, patient safety awareness, and strong interpersonal skills
• Basic knowledge of Medicare/Medicaid considerations in post-acute care settings
