Roles & Responsibilities

• Provide patient-centered physical therapy evaluation and treatment for adults and geriatric patients in a skilled nursing setting
• Develop individualized treatment plans that restore function, mobility, and independence
• Collaborate with nursing, occupational therapy, speech therapy, and care teams to coordinate safe, effective care
• Educate patients and families on home programs, fall prevention, and safe mobility strategies
• Document clinical findings and progress accurately and in a timely manner in the facility EMR
• Participate in care planning meetings and contribute to quality improvement initiatives

Qualifications

• Active CA PT License required
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Experience in SNF or geriatrics preferred
• Strong clinical reasoning and hands-on treatment skills
• Reliable professional communication and teamwork skills

Education

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

• Skilled in mobility assessment, therapeutic exercise, and safe transfer training
• Knowledge of geriatric-specific conditions, wound care basics, and fall risk management
• Proficient with documentation in electronic medical records
• Ability to prioritize caseloads and manage time effectively
• Strong interpersonal skills for working with residents, families, and interdisciplinary teams
