Roles & Responsibilities

• Provide high-quality physical therapy evaluations and create individualized treatment plans for residents
• Deliver evidence-based therapeutic interventions to improve mobility, strength, and independence
• Collaborate with interdisciplinary team members to coordinate resident care and optimize outcomes
• Document therapy sessions and progress notes accurately and in a timely manner
• Communicate goals and progress clearly with residents, families, and facility staff
• Adapt treatment plans to meet changing clinical needs and ensure resident safety

Qualifications

• Current Physical Therapist license in Mississippi or eligibility to obtain state licensure
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy
• Basic Life Support (BLS) / CPR certification preferred
• Prior experience working in skilled nursing or long-term care settings preferred
• Strong communication skills and a resident-centered approach
• Ability to pass background checks and meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, manual therapy, and balance/falls prevention
• Familiarity with documentation standards and electronic medical records
• Ability to assess functional mobility and design measurable goals
• Knowledge of safety protocols for patient transfers and mobility assistance
• Comfort collaborating with nursing and rehab teams in a fast-paced environment
