Roles & Responsibilities
- Assess patients and create individualized occupational therapy plans focused on independence and quality of life
- Deliver evidence-based therapeutic interventions to address ADLs, mobility, cognition, and functional needs
- Collaborate with nursing, PT, SLP, and interdisciplinary team members to coordinate patient care
- Document progress and outcomes accurately and timely in the patient record
- Educate patients and families on safe strategies, adaptive equipment, and home programs
- Monitor and adjust treatment plans based on patient response and goals
Qualifications
- Current Occupational Therapist license in Texas (required)
- Graduation from an accredited Occupational Therapy program (OTR/OT) and active certification
- CPR/BLS certification or ability to obtain upon hire
- Experience in skilled nursing, long-term care, or geriatric populations is preferred
- Strong communication, teamwork, and documentation skills
Education
- Bachelor's or Master's degree in Occupational Therapy (entry-level Master's or OTD preferred)
Knowledge & Skills
- Skilled in ADL retraining, therapeutic activities, and adaptive equipment recommendations
- Familiarity with common SNF workflows and Medicare/Medicaid documentation
- Proficient with clinical documentation and electronic medical records
- Ability to assess functional status, write measurable goals, and track outcomes
- Comfortable teaching patients and caregivers practical, safe strategies
