Roles & Responsibilities

• Perform thorough resident assessments and document findings clearly
• Administer medications safely and manage medication-related tasks
• Provide ADL assistance and person-centered direct care to residents
• Manage gastrostomy tube care and urinary catheter care with attention to infection control
• Deliver nebulized treatments, CPAP support, wound vac care, and other assigned clinical treatments
• Notify MD/NP of pertinent clinical changes and collaborate on care plans
• Admit and discharge residents while ensuring continuity of care and accurate documentation
• Work as part of the interdisciplinary team to promote resident well-being and quality outcomes

Qualifications

• Active LPN license in good standing (state licensure required)
• Experience working in a Long Term Care setting (LTC experience required)
• Current BLS/CPR certification or willing to obtain prior to start
• Proof of flu vaccination or facility-approved medical/religious exemption prior to contract start
• Up-to-date TB screening and employee health clearance as required by facility

Education

• Graduate of an accredited Practical Nursing (LPN) program
• Active LPN license required

Knowledge & Skills

• Strong patient care and resident-centered skills
• Medication administration and documentation proficiency
• ADL support and mobility assistance techniques
• Gastrostomy and catheter care knowledge
• Experience with nebulized treatments, CPAP management, and wound vac care
• Good clinical assessment, communication, and charting skills
• Ability to prioritize, work independently, and collaborate with clinical teams
