Roles & Responsibilities

• Assess patients and develop individualized care coordination and discharge plans that help patients transition safely home or to the next level of care
• Coordinate interdisciplinary family meetings and actively participate in treatment planning to ensure continuity of care
• Facilitate referrals and arrange DME, SNF placements and ambulance transport independently and efficiently
• Document all patient care and care plans accurately in Epic, meeting facility and regulatory standards
• Work with HMOs, IPAs and payers as needed to ensure appropriate care and authorizations
• Support patient populations facing housing instability, behavioral health crises, or complex social needs with compassion and resourcefulness
• Collaborate with nursing and ancillary teams to maintain patient flow and meet unit census needs at a 1:25 ratio

Qualifications

• Current CA state license required (pending license accepted but must be licensed by start date)
• Minimum 2 years acute medical Care Manager experience performing care coordination (HARD STOP — first-timers not accepted)
• Travel/contract experience required
• Epic experience required (computerized documentation is part of daily work)
• Ability to work weekend rotation (every other weekend) and adhere to RTO restrictions (no more than 7 days)
• Strong communication skills and comfort leading family meetings
• Relevant certifications preferred (for example, CCM or other case management credentials)

Education

• BS in Nursing, Social Work, or related field preferred
• Equivalent clinical case management experience will be considered

Knowledge & Skills

• Care coordination, discharge planning and case management best practices
• Experience with arranging DME, SNF and transport logistics
• Familiarity with HMOs, IPAs, DRG, CMS rules and payer processes
• Regulatory knowledge: HIPAA, OSHA and The Joint Commission/Core Measures
• Comfort charting in Epic and working in acute care environments (ED, ICU, MS, Surgical Services preferred)
• Ability to assess social determinants of health, protective services, advance directives and POLST
