Roles & Responsibilities
- Coordinate and manage patient care transitions, ensuring safe and timely discharges
- Perform utilization review and determine medical necessity using evidence-based guidelines
- Document thoroughly in Epic to maintain clear, audit-ready charts
- Facilitate family meetings and collaborate on interdisciplinary treatment plans
- Arrange post-acute resources independently — DME, SNF placement, ambulance transport
- Advocate for patients with payors, providers and community resources
- Maintain appropriate patient ratios (typical days 1:20) while prioritizing quality outcomes
- Use criteria such as InterQual to support utilization decisions and prior authorization needs
Qualifications
- Minimum 2 years acute hospital case management experience performing care coordination and discharge planning
- Proven Epic charting experience (required)
- Travel experience required (can screen candidates who meet experience criteria)
- Ability to float between like units based on departmental needs
- BLS preferred; case management certifications (CCM) or related credentials a plus
- Strong communication skills and ability to lead family meetings and coordinate with interdisciplinary teams
- Able to demonstrate recent clinical experience within the last 2 years (HARDSTOP)
Education
- Clinical background required; active RN license or equivalent clinical license preferred
- BSN or bachelor’s degree in a related field preferred
- Case management certification (CCM) or relevant certifications are a plus
Knowledge & Skills
- Deep knowledge of discharge planning, utilization review and care coordination
- Familiarity with InterQual criteria, DRG, CPT and ICD-10 coding concepts
- Understanding of CMS, The Joint Commission standards, NCQA and HIPAA compliance
- Experience with arranging DME, SNF placement and ambulance/transport logistics
- Skilled in Needs Assessment, Plan of Care development, and prior authorization processes
- Comfort working across settings: acute hospital, behavioral health, home health, LTAC and rehab
