Roles & Responsibilities

• Coordinate patient care across the hospital and community partners to ensure safe, timely discharges
• Conduct utilization review and determine medical necessity using evidence-based guidelines
• Develop and communicate individualized discharge plans, arranging DME, SNF placement and transport as needed
• Document thoroughly and accurately in Epic to maintain high-quality patient records and compliance
• Facilitate family meetings and collaborate with interdisciplinary teams including PT, respiratory, social work and more
• Manage prior authorization tasks and benefits eligibility to remove barriers to patient care

Qualifications

• Minimum 2 years acute hospital case management experience focused on care coordination and discharge planning
• Current work history within the last 2 years in a relevant clinical setting (HARDSTOP requirement)
• Experience with Epic charting system (required)
• Experience with utilization review tools (InterQual) and familiarity with DRG/CPT/ICD-10/NCQA standards
• BLS preferred; other certifications noted on the checklist are a plus
• Travel experience preferred but first-time travelers may be considered for screening with clinical approval

Education

• Not specifically stated; relevant clinical background or healthcare education preferred (e.g., BS/AS in Nursing, Social Work, or related field)

Knowledge & Skills

• Strong case management and care coordination skills
• Utilization review and discharge planning expertise
• Proficiency with Epic electronic health record
• Familiarity with InterQual criteria, medical necessity determination and prior authorization processes
• Knowledge of regulatory and billing concepts: DRG, CPT, ICD-10, HIPAA and The Joint Commission standards
• Ability to arrange DME, SNF placements and ambulance transport independently
• Excellent communication, organization and family meeting facilitation skills
