Roles & Responsibilities
- Coordinate comprehensive care and discharge planning for adult and geriatric patients
- Perform utilization review and determine medical necessity using evidence-based guidelines
- Facilitate interdisciplinary family meetings and contribute to individualized treatment plans
- Arrange DME, SNF placement and ambulance transport independently when needed
- Document thoroughly and accurately in Epic to support quality, compliance and billing
- Collaborate with case managers, social services, PT/OT, IV teams and specialty services to ensure seamless care transitions
- Apply CMS, DRG, ICD-10, CPT and HIPAA knowledge to support regulatory compliance and appropriate reimbursement
Qualifications
- Minimum 2 years acute hospital case management experience performing care coordination and discharge planning in a fast-paced environment
- Travel clinician experience required; locals accepted and first-timers may screen with clinical approval
- Proven experience with Epic electronic health record required
- Current clinical license required; RN preferred or other relevant clinical licensure acceptable
- BLS preferred; other certifications per facility guidelines encouraged
- Comfort managing a patient ratio around 1:20 on day shift and handling a variety of medical and behavioral health diagnoses
- Ability to work 2 weekends every 6 weeks and float between units as needed
Education
- Relevant clinical degree or certification preferred (for example BSN, ADN, or related health degree)
- Equivalent clinical experience will be considered
Knowledge & Skills
- Case management and utilization review principles
- Admission criteria, discharge planning and care coordination
- InterQual criteria and determination of medical necessity
- Familiarity with CPT, DRG, ICD-10 coding and NCQA standards
- HIPAA compliance and The Joint Commission core measures
- Experience with HMOs, IPAs, prior authorizations and benefits eligibility
- Skilled in data abstraction, plan of care development and workers' compensation case management
- Strong communication, family meeting facilitation and interdisciplinary collaboration skills
