Roles & Responsibilities

• Coordinate patient care across the interdisciplinary team to ensure safe, efficient discharges
• Perform thorough discharge planning and arrange DME, SNF and ambulance transport independently
• Lead family meetings and participate in development of individualized treatment plans
• Conduct needs assessments and create clear, patient-centered plans of care
• Document care coordination and case management activities in Epic with accuracy
• Collaborate with HMOs, IPAs and payers to support transition of care and benefits authorization
• Ensure compliance with regulatory requirements such as CMS, DRG, HIPAA and The Joint Commission

Qualifications

• Active CA state license required (pending license accepted but must be active by start date)
• Minimum 2 years acute medical Care Manager experience performing care coordination (hard requirement)
• Travel/contract experience required — first-time travelers not accepted
• Experience with Epic electronic health record required
• Ability to work weekend rotations (every other weekend) and float between facilities
• Strong communication skills and comfort leading family meetings and care conferences
• Familiarity with arranging post-acute care placements, DME and transportation

Education

• Bachelor’s degree in Nursing, Social Work, or a related clinical field preferred; equivalent clinical experience considered

Knowledge & Skills

• Deep knowledge of care coordination, discharge planning and case management workflows
• Familiarity with regulatory frameworks: CMS, DRG, HIPAA, OSHA, The Joint Commission
• Understanding of payer systems, HMOs/IPAs and appeals/denials processes
• Skilled in needs assessment, plan-of-care development and disease management
• Competent in using Epic for documentation and care coordination
• Experience addressing complex social needs: homelessness, substance use, mental health crises, adoption/foster care scenarios
