Roles & Responsibilities
- Coordinate patient care across the interdisciplinary team to ensure safe, efficient discharges
- Perform thorough discharge planning and arrange DME, SNF and ambulance transport independently
- Lead family meetings and participate in development of individualized treatment plans
- Conduct needs assessments and create clear, patient-centered plans of care
- Document care coordination and case management activities in Epic with accuracy
- Collaborate with HMOs, IPAs and payers to support transition of care and benefits authorization
- Ensure compliance with regulatory requirements such as CMS, DRG, HIPAA and The Joint Commission
Qualifications
- Active CA state license required (pending license accepted but must be active by start date)
- Minimum 2 years acute medical Care Manager experience performing care coordination (hard requirement)
- Travel/contract experience required — first-time travelers not accepted
- Experience with Epic electronic health record required
- Ability to work weekend rotations (every other weekend) and float between facilities
- Strong communication skills and comfort leading family meetings and care conferences
- Familiarity with arranging post-acute care placements, DME and transportation
Education
- Bachelor’s degree in Nursing, Social Work, or a related clinical field preferred; equivalent clinical experience considered
Knowledge & Skills
- Deep knowledge of care coordination, discharge planning and case management workflows
- Familiarity with regulatory frameworks: CMS, DRG, HIPAA, OSHA, The Joint Commission
- Understanding of payer systems, HMOs/IPAs and appeals/denials processes
- Skilled in needs assessment, plan-of-care development and disease management
- Competent in using Epic for documentation and care coordination
- Experience addressing complex social needs: homelessness, substance use, mental health crises, adoption/foster care scenarios
