Roles & Responsibilities
- Coordinate patient-centered care and lead discharge planning to ensure safe, timely transitions
- Perform utilization review and determine medical necessity using evidence-based guidelines
- Document thoroughly in Epic and maintain accurate, compliant clinical records
- Arrange DME, SNF placements and ambulance transport independently when needed
- Organize and participate in family meetings and contribute to treatment plan development
- Collaborate with interdisciplinary teams including therapy, respiratory, social services and rapid response
- Perform prior authorizations and work with payers to secure benefits for patients
- Support quality and regulatory compliance activities related to CMS, DRG and other standards
Qualifications
- Minimum 2 years of acute hospital case management experience (care coordination and discharge planning) in a fast-paced environment
- Experience with Epic charting required
- Travel assignment experience required; local clinicians may be considered
- Ability to manage a patient ratio around 1:20 on day shift
- Certifications: BLS preferred; additional case management certifications a plus
- Comfort floating between like units and working with diverse patient populations
- Strong communication, organization and documentation skills
- Ability to pass credentialing, background and meet any RTO restrictions at time of submittal
Education
- Associate or Bachelor degree in Nursing or a related healthcare field preferred
- Clinical license as applicable (RN preferred) or other relevant clinical credential
Knowledge & Skills
- Care coordination, discharge planning and utilization review best practices
- InterQual criteria and determination of medical necessity
- Familiarity with CMS, DRG, CPT and ICD-10 coding fundamentals
- HIPAA compliance, NCQA standards and Joint Commission/core measures awareness
- Experience with DME, SNF placement processes and arranging patient transport
- Data abstraction, needs assessment and plan of care development
- Comfort working in acute hospital specialties such as ED, ICU, MS NICU and behavioral health
