Roles & Responsibilities

• Provide compassionate direct patient care to residents, prioritizing dignity and comfort
• Administer medications and treatments accurately and document thoroughly
• Monitor patient status, report changes promptly, and escalate concerns to the care team
• Assist with activities of daily living including hygiene, mobility, and nutrition support
• Maintain accurate clinical records in the facility EMR and ensure timely charting
• Collaborate with nurses, therapists, and nursing leadership to support individualized care plans
• Support a safe environment through infection control practices and facility policies

Qualifications

• Active LPN license in New Hampshire or ability to obtain prior to start
• Graduate of an accredited practical nursing program
• Current BLS/CPR certification required
• Medication administration experience in long term care or similar setting preferred
• Comfortable working 12-hour night shifts and handling multiple patients
• Professional, reliable, and a team player with strong observational skills

Education

• High school diploma or GED required
• Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Medication administration and documentation accuracy
• Familiarity with long term care protocols and resident-centered care
• Proficient with electronic medical records and basic computer skills
• Excellent communication, teamwork, and time management
• De-escalation and safe patient-handling techniques
