Roles & Responsibilities
- Provide compassionate direct patient care to residents, prioritizing dignity and comfort
- Administer medications and treatments accurately and document thoroughly
- Monitor patient status, report changes promptly, and escalate concerns to the care team
- Assist with activities of daily living including hygiene, mobility, and nutrition support
- Maintain accurate clinical records in the facility EMR and ensure timely charting
- Collaborate with nurses, therapists, and nursing leadership to support individualized care plans
- Support a safe environment through infection control practices and facility policies
Qualifications
- Active LPN license in New Hampshire or ability to obtain prior to start
- Graduate of an accredited practical nursing program
- Current BLS/CPR certification required
- Medication administration experience in long term care or similar setting preferred
- Comfortable working 12-hour night shifts and handling multiple patients
- Professional, reliable, and a team player with strong observational skills
Education
- High school diploma or GED required
- Graduate of an accredited LPN/Vocational Nursing program
Knowledge & Skills
- Strong clinical assessment and basic nursing skills
- Medication administration and documentation accuracy
- Familiarity with long term care protocols and resident-centered care
- Proficient with electronic medical records and basic computer skills
- Excellent communication, teamwork, and time management
- De-escalation and safe patient-handling techniques
