Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
- Administer medications and treatments accurately and on schedule
- Monitor and document patient conditions, reporting changes promptly
- Collaborate with interdisciplinary teams to create and follow personalized care plans
- Perform routine assessments and assist with ADLs to promote resident dignity and comfort
- Educate residents and families on care plans, medications, and follow-up needs
- Support a safe, clean environment by following infection control and facility policies
Qualifications
- Valid Massachusetts LPN license required
- Current BLS/CPR certification (or willing to obtain prior to start)
- Minimum 1 year of LPN experience preferred in long-term care, rehab, or skilled nursing (new grads considered)
- Current immunizations and TB testing per facility policy
- Strong communication skills and a collaborative attitude
- Ability to perform physical duties of the role (assist with transfers, bending, standing for shifts)
Education
- LPN diploma or certificate from an accredited program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and documentation best practices
- Basic wound care and rehab nursing support
- Resident assessment and care planning
- Electronic medical record charting (facility-specific EMR training provided)
- Infection prevention and control procedures
- Team communication and handoff reporting
