Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing/rehab setting
• Administer medications and treatments accurately and on schedule
• Monitor and document patient conditions, reporting changes promptly
• Collaborate with interdisciplinary teams to create and follow personalized care plans
• Perform routine assessments and assist with ADLs to promote resident dignity and comfort
• Educate residents and families on care plans, medications, and follow-up needs
• Support a safe, clean environment by following infection control and facility policies

Qualifications

• Valid Massachusetts LPN license required
• Current BLS/CPR certification (or willing to obtain prior to start)
• Minimum 1 year of LPN experience preferred in long-term care, rehab, or skilled nursing (new grads considered)
• Current immunizations and TB testing per facility policy
• Strong communication skills and a collaborative attitude
• Ability to perform physical duties of the role (assist with transfers, bending, standing for shifts)

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED required

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and rehab nursing support
• Resident assessment and care planning
• Electronic medical record charting (facility-specific EMR training provided)
• Infection prevention and control procedures
• Team communication and handoff reporting
