Roles & Responsibilities
- Provide compassionate, resident-centered direct care to support daily living and comfort
- Administer medications and treatments safely and accurately, following facility policies
- Monitor and document vital signs, changes in condition, and resident progress
- Collaborate with nurses, therapists, and care staff to create and follow care plans
- Maintain clear, timely documentation in the resident record to ensure continuity of care
- Support infection control practices and maintain a safe, clean environment
- Communicate with residents and families with empathy and professionalism
Qualifications
- Active LPN license in Massachusetts (required)
- Graduate of an accredited LPN / Practical Nursing program
- Current BLS/CPR certification (required)
- Minimum of 1 year experience in long-term care or skilled nursing preferred
- Ability to pass background checks and meet facility immunization requirements
- Strong communication skills and a team-oriented attitude
Education
- Graduate of accredited LPN / Practical Nursing program; High school diploma or GED required
Knowledge & Skills
- Medication administration and basic IV/shot experience
- Wound care basics and pressure injury prevention
- Accurate vital signs collection and clinical observation
- Electronic health record documentation and paper charting
- Infection prevention and safe patient handling
- Resident-centered care with strong interpersonal skills
