Roles & Responsibilities

• Provide compassionate, resident-centered direct care to support daily living and comfort
• Administer medications and treatments safely and accurately, following facility policies
• Monitor and document vital signs, changes in condition, and resident progress
• Collaborate with nurses, therapists, and care staff to create and follow care plans
• Maintain clear, timely documentation in the resident record to ensure continuity of care
• Support infection control practices and maintain a safe, clean environment
• Communicate with residents and families with empathy and professionalism

Qualifications

• Active LPN license in Massachusetts (required)
• Graduate of an accredited LPN / Practical Nursing program
• Current BLS/CPR certification (required)
• Minimum of 1 year experience in long-term care or skilled nursing preferred
• Ability to pass background checks and meet facility immunization requirements
• Strong communication skills and a team-oriented attitude

Education

• Graduate of accredited LPN / Practical Nursing program; High school diploma or GED required

Knowledge & Skills

• Medication administration and basic IV/shot experience
• Wound care basics and pressure injury prevention
• Accurate vital signs collection and clinical observation
• Electronic health record documentation and paper charting
• Infection prevention and safe patient handling
• Resident-centered care with strong interpersonal skills
