Roles & Responsibilities
- Conduct thorough speech, language, and swallowing evaluations with compassion and clinical rigor
- Develop individualized treatment plans that target measurable progress and meaningful outcomes
- Deliver evidence-based therapy for communication and dysphagia disorders in a long-term care setting
- Collaborate with nursing, therapy, and family members to ensure continuity of care
- Document assessments and progress accurately and timely in the facility record system
- Coach staff and caregivers on safe swallowing strategies and communication supports
- Participate in care planning meetings and contribute to a person-centered approach
Qualifications
- Current state license as a Speech-Language Pathologist (CO preferred or eligibility)
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
- BLS/CPR certification or willingness to obtain
- Prior experience in long-term care, skilled nursing, or related settings is a plus
- Strong clinical assessment and dysphagia management skills
- Excellent communication skills and a collaborative attitude
Education
- Master's degree in Speech-Language Pathology or equivalent
Knowledge & Skills
- Proficiency in administering standard SLP assessments and swallowing evaluations
- Knowledge of documentation best practices and familiarity with clinical EHRs
- Understanding of Medicare and long-term care regulations and billing basics
- Skilled at training and educating interdisciplinary team members and caregivers
- Ability to prioritize caseload and manage time efficiently
