Roles & Responsibilities

• Conduct thorough speech, language, and swallowing evaluations with compassion and clinical rigor
• Develop individualized treatment plans that target measurable progress and meaningful outcomes
• Deliver evidence-based therapy for communication and dysphagia disorders in a long-term care setting
• Collaborate with nursing, therapy, and family members to ensure continuity of care
• Document assessments and progress accurately and timely in the facility record system
• Coach staff and caregivers on safe swallowing strategies and communication supports
• Participate in care planning meetings and contribute to a person-centered approach

Qualifications

• Current state license as a Speech-Language Pathologist (CO preferred or eligibility)
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
• BLS/CPR certification or willingness to obtain
• Prior experience in long-term care, skilled nursing, or related settings is a plus
• Strong clinical assessment and dysphagia management skills
• Excellent communication skills and a collaborative attitude

Education

• Master's degree in Speech-Language Pathology or equivalent

Knowledge & Skills

• Proficiency in administering standard SLP assessments and swallowing evaluations
• Knowledge of documentation best practices and familiarity with clinical EHRs
• Understanding of Medicare and long-term care regulations and billing basics
• Skilled at training and educating interdisciplinary team members and caregivers
• Ability to prioritize caseload and manage time efficiently
