Roles & Responsibilities
- Assess and evaluate residents to create individualized, evidence-based treatment plans
- Deliver skilled therapeutic interventions focused on mobility, fall reduction, and function
- Collaborate with nursing and interdisciplinary teams to coordinate care and discharge planning
- Document progress and outcomes accurately and timely in the facility EMR
- Educate residents and families on home programs and strategies to maximize independence
- Monitor and modify care plans based on resident response and evolving clinical needs
Qualifications
- Current Georgia physical therapy license (required)
- Doctor of Physical Therapy (DPT) or equivalent required; Master’s considered if licensed
- BLS/CPR certification required
- Experience in skilled nursing, geriatric or dementia care preferred
- Strong clinical assessment, therapeutic exercise and mobility-training skills
- Reliable transportation for travel between sites if choosing a split schedule
Education
- Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy with active GA licensure
Knowledge & Skills
- Skilled in geriatric rehabilitation and dementia-sensitive care approaches
- Excellent documentation and clinical reasoning skills
- Ability to provide safe transfers, gait training and fall prevention strategies
- Strong communication and teamwork with interdisciplinary staff and families
- Comfortable using standard EMR systems and electronic documentation
