Roles & Responsibilities
- Evaluate residents and develop individualized treatment plans that improve mobility, independence, and quality of life
- Deliver evidence-based therapeutic interventions and document progress clearly and professionally
- Collaborate with nursing, occupational therapy, speech therapy, and families to create coordinated care plans
- Educate residents and caregivers on safe mobility techniques and fall prevention
- Discharge plan coordination and timely communication of progress to the interdisciplinary team
- Mentor and support rehab assistants and new staff to maintain a high standard of care
Qualifications
- Active Pennsylvania Physical Therapist license required
- DPT or Master's degree in Physical Therapy preferred
- Current BLS/CPR certification required or willing to obtain
- Experience working in a skilled nursing facility or post-acute care preferred
- Strong clinical assessment, documentation, and communication skills
- Ability to manage a patient caseload and prioritize care in a fast-paced setting
Education
- DPT (Doctor of Physical Therapy) preferred; Master's in Physical Therapy accepted
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, and transfers
- Familiarity with SNF documentation, Medicare guidelines, and progress notes
- Good clinical reasoning and problem-solving with measurable goal setting
- Strong interpersonal skills and a collaborative approach to patient care
- Time management, organization, and efficient documentation habits
