Roles & Responsibilities
- Provide skilled evaluation and treatment to residents to restore mobility and function
- Create individualized care plans and track measurable outcomes
- Deliver evidence-based therapeutic interventions and progress patients toward goals
- Document treatment plans and patient progress accurately in the facility charting system
- Collaborate with nursing, social services, and family members to ensure coordinated care
- Educate patients and caregivers on home exercise programs and safe mobility techniques
- Participate in team meetings to enhance resident care and facility performance
Qualifications
- Active Physical Therapist license in Mississippi or compact licensure eligibility
- Current BLS/CPR certification (or willingness to obtain promptly)
- Experience in skilled nursing, long-term care, or outpatient rehab preferred
- Strong interpersonal and communication skills; compassionate bedside manner
- Ability to manage case load efficiently while maintaining high-quality care
Education
- Doctor of Physical Therapy (DPT) or equivalent degree with state licensure
Knowledge & Skills
- Patient assessment and therapeutic exercise prescription
- Gait training, balance training, and assistive device recommendations
- Familiarity with documentation best practices and EHR/charting systems
- Knowledge of safety protocols for mobility and fall prevention
- Ability to develop measurable, patient-centered treatment plans
