Roles & Responsibilities
- Provide compassionate direct resident care in a skilled nursing and rehab setting
- Administer medications and treatments accurately and according to policy
- Perform timely and accurate documentation in the charting system
- Monitor residents' conditions and report changes to RN leadership and the care team
- Support rehabilitation goals by assisting with mobility, ADLs, and therapy reinforcement
- Collaborate with nurses, therapists, and family members to deliver person-centered care
- Follow infection control, safety, and regulatory protocols to protect residents and staff
Qualifications
- Valid LPN license in Massachusetts (required)
- Current BLS/CPR certification (required or obtain upon hire)
- At least 1 year of recent experience in long-term care or rehab preferred
- Up-to-date immunizations and a negative TB test per facility policy
- Ability to pass background checks and required pre-employment screenings
- Strong communication skills and a team-first attitude
Education
- LPN diploma or certificate from an accredited program; High school diploma or GED
Knowledge & Skills
- Medication administration and basic nursing procedures
- Resident assessment and documentation best practices
- Familiarity with long-term care and rehabilitation workflows
- Basic EMR/charting skills and attention to detail
- Infection prevention, safety, and emergency response principles
- Compassionate patient interaction and family communication
