Roles & Responsibilities
- Provide compassionate, high-quality nursing care to homebound patients
- Administer medications and treatments accurately and safely
- Perform focused assessments and document clinical findings in the chart
- Collaborate with the care team to create and follow individualized care plans
- Educate patients and families on care routines, medications, and safety
- Report changes in patient condition promptly to the supervising RN or provider
- Maintain professional, timely communication with patients, families, and the team
Qualifications
- Active LPN license in Maryland (required)
- Current BLS/CPR certification (required)
- Minimum 1 year of LPN clinical experience preferred; home health experience a plus
- Up-to-date immunizations and TB test per employer policy
- Ability to pass background check and drug screen
- Reliable transportation to travel within the Homewood/Baltimore service area
Education
- LPN diploma or vocational nursing certificate
Knowledge & Skills
- Strong medication administration and documentation skills
- Comfortable with wound care, vital signs, and basic skilled tasks
- Familiarity with electronic medical records (EMR) and timely charting
- Excellent patient education and interpersonal communication
- Ability to work independently in patient homes and problem-solve on the spot
