Roles & Responsibilities
- Provide compassionate direct patient care including vital signs, bathing, feeding and mobility assistance
- Administer medications and document administration accurately and timely
- Monitor and report changes in patient condition to the care team to support great outcomes
- Assist with wound care and routine treatments per care plans
- Maintain accurate, concise electronic and paper charting
- Collaborate with nurses, therapists and family members to deliver person-centered care
- Adhere to infection control and safety protocols to protect residents and staff
Qualifications
- Active Maryland LPN license in good standing
- Current BLS/CPR certification
- Preferred: experience in long-term care, skilled nursing or similar settings
- Ability to work a 12+ hour day shift and provide dependable coverage
- Up-to-date immunizations, TB test and ability to pass background check and drug screen
- Strong communication skills and a team-first attitude
Education
- Graduate of an accredited LPN/LVN program or equivalent
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and dressing changes
- Assessment of vital signs and recognition of changes in condition
- Comfortable with electronic charting and timely documentation
- Time management and prioritization during busy shifts
- Clear communication with interdisciplinary teams and families
