Roles & Responsibilities
- Evaluate patients' functional abilities and create individualized treatment plans
- Deliver skilled occupational therapy interventions to support independence in daily living
- Collaborate closely with nursing, PT, SLP and the interdisciplinary team for coordinated care
- Document patient progress accurately and timely in the clinical record
- Educate patients and families on home strategies and safety to promote long-term outcomes
- Recommend adaptive equipment and environmental modifications to maximize function
- Participate in team meetings and discharge planning to ensure smooth transitions
Qualifications
- Current California occupational therapy license (required)
- OTR/L credential preferred
- Basic Life Support (BLS) / CPR certification (current)
- Minimum 1 year of recent OT experience in post-acute, skilled nursing, or inpatient rehab settings preferred
- Strong clinical assessment and documentation skills
- Ability to work collaboratively within an interdisciplinary team
Education
- Master's degree in Occupational Therapy (MOT/OTD) or equivalent entry-level degree
Knowledge & Skills
- Skilled in ADL/IADL assessment and treatment planning
- Knowledge of assistive devices, splinting basics, and positioning techniques
- Familiarity with post-acute care workflows and discharge planning
- Effective verbal and written communication with patients, families and staff
- Time management and caseload prioritization in a fast-paced setting
- Comfortable using electronic medical records for documentation
