Roles & Responsibilities

• Evaluate patients' functional abilities and create individualized treatment plans
• Deliver skilled occupational therapy interventions to support independence in daily living
• Collaborate closely with nursing, PT, SLP and the interdisciplinary team for coordinated care
• Document patient progress accurately and timely in the clinical record
• Educate patients and families on home strategies and safety to promote long-term outcomes
• Recommend adaptive equipment and environmental modifications to maximize function
• Participate in team meetings and discharge planning to ensure smooth transitions

Qualifications

• Current California occupational therapy license (required)
• OTR/L credential preferred
• Basic Life Support (BLS) / CPR certification (current)
• Minimum 1 year of recent OT experience in post-acute, skilled nursing, or inpatient rehab settings preferred
• Strong clinical assessment and documentation skills
• Ability to work collaboratively within an interdisciplinary team

Education

• Master's degree in Occupational Therapy (MOT/OTD) or equivalent entry-level degree

Knowledge & Skills

• Skilled in ADL/IADL assessment and treatment planning
• Knowledge of assistive devices, splinting basics, and positioning techniques
• Familiarity with post-acute care workflows and discharge planning
• Effective verbal and written communication with patients, families and staff
• Time management and caseload prioritization in a fast-paced setting
• Comfortable using electronic medical records for documentation
