Roles & Responsibilities
- Provide safe, compassionate LPN care to residents including medication administration and treatments
- Perform accurate and timely documentation in the resident chart and EHR
- Monitor resident condition, record vital signs, and report changes to RN/charge nurse
- Assist with ADLs and promote resident dignity, independence, and comfort
- Collaborate with the interdisciplinary care team to implement individualized care plans
- Support infection control and facility safety procedures to protect residents and staff
- Communicate clearly and professionally with residents, families, and team members to ensure continuity of care
Qualifications
- Active LPN license in Alabama required
- Completion of an accredited Practical Nursing program
- Current BLS/CPR certification required (or willingness to obtain prior to start)
- Prefer 1+ year of recent LPN experience in a skilled nursing or long-term care setting
- Must be able to pass background check and drug screen
- Immunizations up to date per facility policy
Education
- Graduate of an accredited Practical Nursing program; LPN licensure required
Knowledge & Skills
- Medication administration and documentation accuracy
- Vital signs assessment and recognition of clinical changes
- Proficient charting in an electronic health record (EHR)
- Strong communication and teamwork skills
- Basic wound care and ADL assistance
- Time management and ability to prioritize in a fast-paced setting
