Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents
- Administer medications and treatments accurately and safely
- Monitor and document vital signs, changes in condition, and care provided
- Collaborate with nurses, aides, and interdisciplinary team members to implement care plans
- Perform timely and accurate documentation in the resident record
- Support residents with activities of daily living while promoting independence and dignity
- Ensure compliance with infection control and safety protocols
- Assist with admissions, transfers, and discharges when needed
Qualifications
- Current LPN license in New Hampshire (required)
- T-LNA or LNA certification is a plus
- Current BLS/CPR certification (or willingness to obtain upon hire)
- Minimum 1 year of LPN experience preferred, long-term care or skilled nursing experience a plus
- Ability to pass background check and drug screen
- Strong communication skills and a team-oriented attitude
Education
- Graduate of an accredited LPN/Vocational Nursing program
Knowledge & Skills
- Medication administration and accurate documentation
- Patient assessment and monitoring
- Basic wound care and infection control practices
- Ability to follow care plans and escalate changes appropriately
- Time management and prioritization during busy shifts
- Professional, compassionate resident-centered communication
