Roles & Responsibilities
- Administer medications and treatments safely and accurately following facility policy
- Perform routine resident assessments and document changes in condition promptly
- Collaborate with RNs and the interdisciplinary team to develop and follow care plans
- Assist residents with activities of daily living while promoting dignity and independence
- Communicate clearly with families and team members to ensure coordinated, compassionate care
- Maintain accurate, timely documentation in electronic or paper health records
- Adhere to infection control, safety, and regulatory standards at all times
Qualifications
- Active LPN license in New Jersey (required)
- Current BLS/CPR certification (required or obtain upon hire)
- Previous experience in long-term care or skilled nursing preferred
- Strong clinical assessment, medication administration, and documentation skills
- Ability to work every-other-weekend rotation as noted
Education
- Diploma or certificate from an accredited Practical Nursing program
- Successful completion of NCLEX-PN and active state licensure
Knowledge & Skills
- Medication administration and safe charting practices
- Basic wound care and monitoring for changes in resident status
- Familiarity with long-term care workflows and regulatory requirements
- Effective communication with residents, families, and care teams
- Time management and prioritization in a fast-paced environment
