Roles & Responsibilities
- Manage a panel of SEARHC patients by coordinating and actively overseeing their care
- Collaborate with medical providers and interdisciplinary teams to set individualized medical goals
- Coordinate multi-disciplinary care conferences for high-risk or complex patients
- Perform nursing assessments, documentation, patient education, and medication administration
- Track and manage referrals and waitlists, ensuring necessary diagnostics and records are complete
- Serve as primary case manager for OB patients, including intake, outreach, and postpartum follow-up
- Support specialty clinics by preparing clinical space, assisting providers during procedures, and scheduling
- Coordinate and act as nursing staff for video teleconferencing (VTC) visits with specialty providers
- Identify obstacles to care and proactively work to resolve issues and improve processes
- Maintain annual competencies, infection control practices, and participate in quality improvement
Qualifications
- Current full, unrestricted Alaska Registered Nurse license required at time of submittal
- Minimum 1 year of case management experience, with case manager experience within the last 2 years
- Graduate of an accredited school of nursing; Bachelor's degree preferred
- High school diploma or equivalent required
- Basic Life Support (BLS) certification preferred
- National Case Management certification preferred
- Proof of flu vaccination indicated by employer; fingerprint cards required before start
- Ability to multi-task in a fast-paced clinical environment and work independently
Education
- Graduate of an accredited school of nursing (required)
- High school diploma or equivalent (required)
- Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Strong assessment skills and application of the nursing process
- Care coordination, referral management, and patient advocacy
- Proficient with EHR systems, documentation, and clinical data tracking
- Triage experience and comfort responding to urgent or emergent situations
- Excellent oral and written communication and patient education skills
- Familiarity with SEARHC and tribal health services in Alaska is a plus
- Ability to use clinical equipment and standard office technology
- Infection control, safety principles, and evidence-based nursing practices
