Roles & Responsibilities

• Assess residents and develop individualized nursing care plans
• Implement nursing interventions and ensure continuity of care under RN supervision
• Administer medications accurately and safely on the night shift
• Supervise CNAs and support staff to maintain high-quality resident care
• Document resident status and care provided in the Electronic Medical Record
• Communicate clearly and compassionately with residents, families, and team members
• Cross-train in other nursing departments as needed to support facility operations
• Participate in shift handoffs to ensure seamless patient care

Qualifications

• Current California LVN license required
• Skilled Nursing Facility (SNF) experience required; Sub-Acute does not count
• Current BLS certification required
• Must pass the facility nursing medical test
• Strong English communication skills, verbal and written
• Ability to supervise CNAs and lead on-shift care activities
• Basic computer skills and comfort using Electronic Medical Records

Education

• Vocational nursing diploma or LVN program graduate (or equivalent) with California LVN licensure

Knowledge & Skills

• Patient assessment and care planning in long-term care
• Medication administration and safe narcotic handling
• Skilled Nursing care practices specific to SNFs
• Experience with Electronic Medical Records is preferred
• BLS life support knowledge and emergency response
• Good interpersonal skills and professional communication
