Roles & Responsibilities

• Provide skilled nursing care for acute inpatient rehab patients, focusing on recovery and functional improvement
• Start and maintain IVs, manage central lines (PICC), and safely administer blood and blood products
• Deliver tracheostomy care, manage feeding tubes and support respiratory therapies like incentive spirometry and supplemental oxygen
• Assess and prevent falls and pressure ulcers through vigilant monitoring and proactive interventions
• Perform patient teaching to promote independence and safe discharge planning
• Collaborate with PT, OT, respiratory therapy, pharmacy and the interdisciplinary team to coordinate care
• Accurately document patient care and progress in Epic to ensure continuity and quality
• Support mobilization techniques and assistive-device use to maximize patient outcomes

Qualifications

• Active Registered Nurse license (North Carolina) or eligible to practice per facility guidelines (pending licenses considered per policy)
• Minimum 4 years of nursing experience in relevant settings; acute inpatient rehab or skilled nursing experience is a strong asset
• BLS certification (preferred); additional specialty certifications welcome
• Previous charge or leadership experience preferred but not required
• Comfortable with team nursing model and working within interdisciplinary rehab services

Knowledge & Skills

• Proficiency with Epic charting system
• IV therapy expertise including starting/maintaining IVs and PICC care
• Feeding tube and PEG care and management
• Tracheostomy care and respiratory monitoring including pulse oximetry and suctioning
• Foley catheter removal and genitourinary care
• Mobility and transfer assistance, safe use of lift/transfer devices, and mobilization techniques
• Fall risk and pressure ulcer assessment/prevention strategies
• Experience with cardiac meds, anticoagulants, seizure management, and post-op orthopedics (total joint) and neuro diagnoses such as CVA and TBI
• Infection prevention, isolation precautions, and safe administration of blood products
