Candidate Pay Rate: $64 per hour

Roles & Responsibilities
- Perform thorough patient assessments and develop individualized plans of care
- Deliver evidence-based acute care and acute rehab treatments to improve function
- Document evaluations, daily notes, reassessments and discharge summaries in Epic
- Collaborate with nursing, respiratory, social work and pharmacy to coordinate care
- Assist with safe patient mobilization including Hoyer lifts, gait belt use, and transfers
- Provide patient and family education to support recovery and discharge planning
- Monitor vital signs and adjust treatment plans for medically complex patients
- Participate in interdisciplinary rounds and contribute to quality improvement
Qualifications
- Current State Physical Therapy license (State licensed required)
- Minimum 3 years of professional experience (with at least 1 year acute care hospital experience preferred)
- BLS certification required
- Proficiency with Epic charting is required
- Comfort and willingness to float between like units with orientation provided
- Experience with acute rehab, mobility training, Hoyer lifts, and wheelchair assessments
- Ability to work the assigned weekend schedule as noted
Knowledge & Skills
- Assessment, documentation, plan of care, reassessments and discharge planning
- Vital sign monitoring and medically complex patient management
- Lifting/transfer techniques, Hoyer lift operation, gait belt and wheelchair assessment
- Orthopedics, neurology, cardiopulmonary and prosthetic training fundamentals
- Modalities familiarity (cryotherapy, moist heat) and musculoskeletal techniques (McKenzie, myofascial release)
- Strong time management, prioritization, and interdisciplinary communication skills
- Experience in acute care and acute rehab settings; comfortable with varied caseloads
