Roles & Responsibilities
- Provide high-quality physical therapy evaluations and hands-on treatments for residents
- Develop and implement individualized therapy plans that improve mobility, safety, and quality of life
- Collaborate closely with the rehab team, nursing staff, and other healthcare partners to coordinate care
- Document skilled services accurately using Net Health (training available)
- Support a resident-centered culture by coaching staff and family and promoting functional independence
- Monitor outcomes and adjust treatment plans to maximize resident progress
Qualifications
- PA Physical Therapist license in good standing (required)
- Current CPR certification through the American Heart Association or Red Cross
- Two-step PPD (TB skin test) within 30 days of start date
- Covid vaccination records or documented exemption
- Background check and fingerprinting clearance
- Resume and a recent physical performed by a physician, PA, or nurse practitioner
- Experience in senior care or working across skilled nursing, personal care, and independent living (preferred)
- Strong communication and teamwork skills; compassionate, resident-focused approach
Education
- Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy (typical requirement for licensure)
Knowledge & Skills
- Physical therapy evaluations and treatment planning
- Therapeutic interventions to improve mobility and safety
- Care plan development and resident-centered goal setting
- Clinical documentation best practices (Net Health experience helpful)
- Strong verbal and written communication with residents, families, and staff
- Ability to work collaboratively within an interdisciplinary rehab team
