Roles & Responsibilities

• Lead and facilitate engaging activities that promote socialization, cognition, and well-being for elderly or disabled participants
• Assist participants with safe participation in group and individual activities, ensuring dignity and comfort
• Collaborate with the interdisciplinary care team to align activities with care plans and participant goals
• Document participant attendance, progress, and any notable changes in status accurately and promptly
• Create a welcoming, safe environment and help maintain activity spaces and supplies
• Encourage participant independence and meaningful engagement through thoughtful program delivery

Qualifications

• High school diploma or GED required
• One year of experience in a patient activities program with the elderly or disabled in a health care setting preferred
• Bilingual (English/Spanish) preferred but not required — bilingual skills are a plus
• Friendly, patient demeanor and strong interpersonal skills to connect with participants and families

Education

• High school diploma or GED

Knowledge & Skills

• Experience planning and running activities for older adults or individuals with disabilities
• Strong communication skills and ability to work as part of a care team
• Basic documentation skills and attention to detail
• Ability to support mobility and participation safely and compassionately
• Cultural sensitivity and an engaging, creative approach to programming
