Roles & Responsibilities
- Assess speech, language, cognitive-communication, and swallowing function in SNF residents
- Develop and implement individualized therapy plans to improve patient outcomes
- Deliver evidence-based treatment and adapt interventions to each resident's needs
- Collaborate with nurses, therapists, physicians, and family members to coordinate care
- Document progress and maintain accurate, timely clinical records
- Educate residents and caregivers on strategies to maximize communication and safety
- Participate in care planning and interdisciplinary meetings to advocate for patients
Qualifications
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) required or in progress
- Valid NJ SLP license required or eligible to obtain state licensure
- Open to all experience levels including new graduates (years of experience: any)
- Current BLS/CPR certification preferred
- Strong clinical assessment and documentation skills in a skilled nursing/rehab setting
Education
- Master's degree in Speech-Language Pathology preferred
Knowledge & Skills
- Skilled in dysphagia evaluation and management
- Experience with cognitive-communication and language therapy in adults
- Familiarity with SNF rehabilitation workflows and interdisciplinary collaboration
- Comfortable with timely electronic documentation and care planning
- Excellent communication, empathy, and patient-centered care approach
