Roles & Responsibilities
- Conduct comprehensive patient assessments and develop individualized PT care plans during home visits
- Manage a full home health caseload averaging 5-6 visits per day with clinical ownership
- Perform admissions, recertifications, and return-on-care evaluations to ensure continuity and quality
- Supervise and mentor PTA staff while overseeing delegated treatment and documentation
- Document thoroughly and timely in the EMR (Cerner Roadnotes) to maintain clear communication with the care team
- Coordinate with physicians and other providers to optimize patient outcomes and ensure appropriate follow-up
Qualifications
- Minimum 2 years of clinical experience with Home Health experience required
- Current BLS certification (required)
- Experience supervising PTAs and managing a home health caseload
- Comfortable using Cerner Roadnotes or similar EMR for documentation
- Valid Physical Therapist license (state requirements apply; ability to practice in NC preferred)
- Strong autonomous work style with excellent organization and communication skills
Education
- Graduation from an accredited Physical Therapy program (per state licensure requirements)
- Doctor of Physical Therapy (DPT) or entry-level PT degree as required by employer/state
Knowledge & Skills
- Skilled in conducting functional assessments, admissions, recertifications, and ROC evaluations
- Proven ability to manage visits and triage patient needs in a home setting
- Experience with care coordination and case management
- Proficient documentation skills and familiarity with laptop and mobile charting (iPhone supported)
- Ability to prioritize workload and problem-solve independently while representing the organization professionally
