Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents according to plan of care
- Administer medications and treatments accurately and safely
- Monitor and document vital signs, symptoms, and resident responses
- Communicate clearly with residents, families, and the interdisciplinary team
- Collaborate with RNs and other clinicians to support quality outcomes
- Maintain accurate and timely electronic health record documentation
Qualifications
- Current LPN license in Pennsylvania (required)
- Completion of an accredited LPN/LVN program
- Basic Life Support (BLS/CPR) certification — current or obtained upon hire
- Strong communication and teamwork skills
- Prior long-term care or skilled nursing experience is a plus
Education
- Completion of an accredited LPN/LVN program; high school diploma or equivalent
Knowledge & Skills
- Medication administration and documentation best practices
- Vital signs assessment and basic clinical observations
- Patient-centered care and infection control principles
- Comfortable using electronic health records
- Time management and prioritization in a fast-paced setting
