Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in line with care plans
- Administer medications and treatments accurately and safely
- Monitor and document patients' conditions, reporting changes to the care team promptly
- Collaborate with nurses, therapists, and physicians to deliver coordinated care
- Assist with activities of daily living while promoting dignity and independence
- Support infection control practices and maintain a clean, safe environment
Qualifications
- Current LPN license in Pennsylvania (required)
- Completion of an accredited Practical Nursing program
- Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
- Comfortable with medication administration, wound care basics, and charting
- Professional, reliable, and patient-focused attitude; great communication skills
Education
- Graduate of an accredited Practical Nursing (LPN/LVN) program
Knowledge & Skills
- Skilled in clinical assessments and vital signs monitoring
- Knowledge of medication administration and documentation standards
- Familiarity with electronic charting or willingness to learn
- Strong interpersonal skills for working with residents and multidisciplinary teams
- Time management and the ability to prioritize patient needs
