Roles & Responsibilities
- Provide compassionate, patient-centered nursing care within scope of LPN practice
- Administer medications and document accurately and promptly
- Monitor patient status and report changes to the RN or provider
- Perform routine clinical tasks such as wound care, vital signs, and IV assistance as allowed
- Collaborate with the care team to develop and follow individualized care plans
- Educate patients and families on care procedures and discharge instructions
- Maintain accurate, timely charting and comply with facility policies
Qualifications
- Current LPN license in Pennsylvania (required)
- Graduate of an accredited LPN or vocational nursing program
- Current BLS/CPR certification or willingness to obtain
- Clear background check and ability to meet facility immunization requirements
- Strong communication skills and a team-oriented mindset
Education
- Graduate of an accredited LPN or vocational nursing program
Knowledge & Skills
- Solid clinical assessment and basic nursing procedure skills
- Medication administration and documentation proficiency
- Basic wound care and vital sign monitoring
- Comfortable using electronic health records and following care plans
- Compassionate patient interaction and effective communication with families
