Roles & Responsibilities
- Assess patients and perform thorough evaluations to determine individualized therapy needs
- Develop and implement tailored treatment plans to improve mobility, strength, and function
- Provide hands-on care including therapeutic exercise, gait training, and manual techniques
- Document patient progress accurately and timely in the medical record
- Collaborate with nurses, clinicians, and care teams to coordinate safe, effective care
- Educate patients and families on home programs, safety, and strategies to maintain gains
- Promote a positive, person-centered environment that supports recovery and dignity
Qualifications
- Active Physical Therapist license in New Hampshire or eligibility to obtain one
- Graduated from an accredited Physical Therapy program (DPT preferred)
- Current BLS/CPR certification or willingness to obtain
- Comfortable working with skilled nursing residents and interdisciplinary teams
- Strong clinical reasoning, documentation skills, and excellent communication
- Prior experience in post-acute, skilled nursing, or geriatric rehab is a plus
Education
- Doctor of Physical Therapy (DPT) from an accredited program preferred
- Or equivalent Physical Therapy degree and state licensure
Knowledge & Skills
- Skilled in assessment, therapeutic exercise, and functional mobility training
- Experience with gait training, transfers, balance and fall prevention strategies
- Familiarity with common rehab modalities and assistive device recommendations
- Proficient clinical documentation and progress note writing
- Team-focused communication and patient/family education abilities
- Ability to prioritize caseload and deliver efficient, high-quality care
