Roles & Responsibilities
- Provide hands-on and evidence-based physical therapy interventions to residents
- Assess functional status, develop goals, and create individualized treatment plans
- Document patient progress clearly and timely in the medical record
- Collaborate with nurses, physicians, and other rehab staff to coordinate care
- Educate residents and family members on safe mobility, exercises, and fall prevention
- Support discharge planning and recommend adaptive equipment when appropriate
Qualifications
- Current Physical Therapist Assistant (PTA) license or certification required (PA licensure preferred)
- Current Basic Life Support (BLS/CPR) certification
- Prior experience in skilled nursing, long-term care, or rehab settings is highly desirable
- Strong interpersonal skills and a patient-centered approach
- Ability to document thoroughly and use electronic medical records
Education
- Associate Degree in Physical Therapist Assistant or related accredited program
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, transfers, and balance training
- Knowledge of safety principles for mobility and fall prevention
- Ability to adapt interventions for varying cognitive and medical needs
- Good clinical reasoning, time management, and teamwork skills
