Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans to improve mobility and function
• Provide skilled therapeutic interventions and document clinical progress clearly and timely
• Collaborate with nursing and interdisciplinary teams to coordinate care and ensure smooth transitions
• Educate residents and families on therapeutic goals, home programs, and safety strategies
• Use outcome measures to track progress and adjust treatment plans for best results
• Maintain professional recordkeeping and comply with facility and regulatory requirements

Qualifications

• Active Physical Therapist license in New Mexico (required)
• Doctor of Physical Therapy (DPT) or equivalent degree in physical therapy
• Current Basic Life Support (BLS) / CPR certification
• Experience in skilled nursing facility or rehabilitation settings preferred
• Strong clinical assessment skills and ability to manage a varied caseload
• Clear background check and ability to meet facility onboarding requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor's or Master's in Physical Therapy acceptable

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, balance and mobility interventions
• Familiarity with long-term care and post-acute care documentation practices
• Ability to use clinical outcome measures and create measurable treatment goals
• Excellent communication and collaboration skills with interdisciplinary teams
• Time management and caseload prioritization in a busy clinical setting
