Roles & Responsibilities
- Coordinate and facilitate safe, timely discharges by arranging DME, SNF placement, and ambulance transport when needed
- Assess patients for appropriate level of care using InterQual criteria and collaborate with the care team to develop treatment plans
- Lead family meetings and communicate discharge plans clearly to patients, families, and community partners
- Document thoroughly and accurately in Epic and other computerized systems to meet regulatory and facility standards
- Apply CMS/DRG knowledge to support appropriate utilization and documentation
- Manage complex patients including those with substance use, mental health crises, or housing instability, connecting them with community resources
Qualifications
- California state license required (pending license accepted but must be active by start date)
- Minimum 2 years acute care hospital case management experience performing care coordination and discharge planning
- Travel experience required; first-time travelers may be screened (first-timers accepted)
- Proficient with Epic and computerized documentation
- Familiarity with InterQual criteria, CMS/DRG rules, and arranging post-acute services (DME, SNF, ambulance)
- Willingness to work weekend rotation and meet department scheduling needs
- Strong communication, organization, and interdisciplinary collaboration skills
- Certifications preferred (listed as a bonus, not required)
Knowledge & Skills
- Acute care hospital case management and discharge planning
- Care coordination across disciplines and community partners
- InterQual utilization criteria and regulatory CMS/DRG concepts
- Epic electronic health record and computerized documentation
- Arranging durable medical equipment, SNF placement, and medical transport
- Experience with HMOs, IPAs, and working within payer processes
- Working with patients affected by substance use, homelessness, and mental health crises
- Proficient in Microsoft Office for documentation and reporting
