Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized plans of care for adult and geriatric patients in an acute/acute rehab setting
• Deliver manual therapy and hand therapy interventions, including use of dynamic and static splints and prosthetics/orthotics approaches when appropriate
• Document thoroughly and efficiently in Epic to support continuity of care, billing, and quality measures
• Collaborate with nursing, respiratory therapy, social services, pharmacy and other disciplines as part of interdisciplinary rounds and discharge planning
• Perform safe lifting and transfer techniques, instruct patients and families on safe mobility and fall prevention
• Use standardized assessments (FIM, cognitive assessments, adaptive equipment assessments) to track progress and modify treatment plans
• Participate in pre-discharge planning and patient education to optimize outcomes and reduce readmissions

Qualifications

• Current, active Physical Therapist license in California (pending licenses are not accepted)
• Minimum 1 year acute care hospital experience (acute rehab experience highly relevant)
• Experience with Epic charting required
• Certifications in Manual Therapy and Hand Therapy indicated in the job details
• BLS/Basic Life Support preferred (certs marked with * noted as preferred)
• Comfort floating between units (acute rehab and acute care) as needed
• Strong communication skills and ability to work collaboratively on interdisciplinary teams

Knowledge & Skills

• Hands-on manual therapy and hand therapy techniques
• Prosthetics/orthotics knowledge (above-knee, below-knee, upper extremity prosthetics)
• Splinting skills: dynamic, functional, static splints
• Modalities such as electrical stimulation, biofeedback, edema massage
• Standardized outcome measures (FIM, cognitive assessments)
• Discharge planning, ADL training, adaptive equipment assessment
• Safety and injury prevention, safe patient handling and transfers
• Familiarity with acute neurology diagnoses (CVA, TBI, spinal cord injury, reattachment)
