Roles & Responsibilities

• Coordinate patient care across interdisciplinary teams to ensure smooth transitions and quality outcomes
• Conduct discharge planning and needs assessments, including ordering DME when needed
• Perform utilization reviews, continued stay reviews and prior authorization follow-ups
• Determine benefits eligibility and apply admission criteria to support appropriate care placement
• Document thoroughly and accurately in Epic to ensure continuity of care and regulatory compliance
• Collaborate with social services and community resources to remove barriers to safe discharge

Qualifications

• Minimum 1 year of relevant experience (documented charge or acute care experience required)
• Proven experience with patient ratios around 1:20–1:25 in an acute hospital setting
• Epic charting experience required
• BLS preferred
• Case Management Certification preferred
• Must have a reliable vehicle for this assignment and be able to float between units/facilities as needed
• Travel experience is not required but flexibility to float is expected

Knowledge & Skills

• Care coordination and discharge planning
• Prior authorizations and utilization review processes
• Continued stay reviews and admission criteria application
• Plan of care development and needs assessment / DME ordering
• Benefits eligibility determination
• Familiarity with CMS, DRG, HEDIS, HIPAA, NCQA, OSHA and The Joint Commission standards
• Experience documenting in Epic
• Comfort working in acute hospital environments and collaborating across teams
