Roles & Responsibilities
- Coordinate patient care across interdisciplinary teams to ensure smooth transitions and quality outcomes
- Conduct discharge planning and needs assessments, including ordering DME when needed
- Perform utilization reviews, continued stay reviews and prior authorization follow-ups
- Determine benefits eligibility and apply admission criteria to support appropriate care placement
- Document thoroughly and accurately in Epic to ensure continuity of care and regulatory compliance
- Collaborate with social services and community resources to remove barriers to safe discharge
Qualifications
- Minimum 1 year of relevant experience (documented charge or acute care experience required)
- Proven experience with patient ratios around 1:20–1:25 in an acute hospital setting
- Epic charting experience required
- BLS preferred
- Case Management Certification preferred
- Must have a reliable vehicle for this assignment and be able to float between units/facilities as needed
- Travel experience is not required but flexibility to float is expected
Knowledge & Skills
- Care coordination and discharge planning
- Prior authorizations and utilization review processes
- Continued stay reviews and admission criteria application
- Plan of care development and needs assessment / DME ordering
- Benefits eligibility determination
- Familiarity with CMS, DRG, HEDIS, HIPAA, NCQA, OSHA and The Joint Commission standards
- Experience documenting in Epic
- Comfort working in acute hospital environments and collaborating across teams
