Roles & Responsibilities
- Evaluate patients to develop individualized treatment plans that improve mobility and independence
- Deliver skilled therapeutic interventions and document progress clearly and promptly
- Collaborate with nurses, physicians, and rehab team members to coordinate patient care
- Educate patients and families on home exercise programs and safe mobility techniques
- Monitor and adjust treatments based on patient response and goals to maximize outcomes
- Contribute to a positive, patient-centered culture that respects dignity and recovery
Qualifications
- Valid Massachusetts Physical Therapist license (required)
- Current CPR/BLS certification
- Previous experience in skilled nursing, long-term care, or rehab preferred
- Strong clinical assessment and documentation skills
- Excellent communication and teamwork abilities
Education
- Doctor of Physical Therapy (DPT) preferred or equivalent degree in Physical Therapy
Knowledge & Skills
- Skilled in therapeutic exercise, gait training, balance and fall prevention
- Knowledge of documentation best practices and electronic medical records
- Ability to create measurable, patient-focused treatment goals
- Familiarity with safety protocols and infection control in a care facility
