Roles & Responsibilities
- Provide compassionate bedside care to residents in a long-term care setting
- Administer medications and treatments safely and per physician orders
- Monitor and document resident conditions, vital signs, and care provided
- Support activities of daily living and promote resident independence
- Collaborate with nursing staff, therapists, and interdisciplinary team to deliver coordinated care
- Educate residents and families about care plans and health needs
- Maintain infection control standards and facility policies
- Respond promptly to changes in resident condition and emergencies
Qualifications
- Current LPN license in Vermont required
- CPR/BLS certification required (or obtain upon hire as permitted)
- Recent LPN experience in long-term care or skilled nursing preferred
- Ability to pass background check and required health screenings
- Strong communication skills, reliability, and a resident-centered attitude
Education
- Graduation from an accredited LPN/practical nursing program
- High school diploma or GED required
Knowledge & Skills
- Medication administration and documentation
- Resident assessment, vital signs, and basic clinical procedures
- Experience with ADL assistance and mobility support
- Familiarity with long-term care regulations and infection control
- Comfortable using electronic health records and accurate charting
- Teamwork, time management, and strong interpersonal skills
