Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in a skilled rehab setting
- Administer medications and treatments accurately and on schedule
- Collaborate with the interdisciplinary team to develop and follow individualized care plans
- Perform thorough assessments and document changes in condition promptly
- Assist with wound care, vital signs monitoring, and other clinical tasks
- Communicate clearly and kindly with residents and families to support excellent patient experience
Qualifications
- Active LPN license in Maryland (required)
- Current BLS/CPR certification (required)
- Experience working in long-term care or rehabilitation preferred
- Strong clinical judgment and the ability to prioritize resident needs
- Excellent communication and teamwork skills
- Ability to pass background checks and required facility screenings
Education
- Vocational/Practical Nursing diploma or equivalent required
- High school diploma or GED preferred
Knowledge & Skills
- Solid knowledge of medication administration and safety
- Familiarity with charting and electronic health records
- Understanding of infection control and wound care principles
- Skill in resident assessment and care planning
- Able to work effectively within a care team and follow clinical protocols
