Roles & Responsibilities
- Provide compassionate direct patient care and monitor patient status
- Administer medications and treatments as ordered, following facility protocols
- Document patient observations, interventions, and responses accurately
- Collaborate with the care team to create and update individualized care plans
- Assist with patient admissions, discharges, and transfers smoothly
- Educate patients and families about care, safety, and follow-up needs
Qualifications
- Active Licensed Practical Nurse (LPN) license in Maryland
- Current BLS/CPR certification (or willingness to obtain)
- Comfortable with medication administration, wound care, and basic nursing procedures
- Strong communication and teamwork skills
- Preferred: prior LTC or facility-based nursing experience
Education
- Completion of an accredited LPN program or equivalent certification
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and documentation best practices
- Patient safety, infection control, and charting systems
- Effective communication with patients, families, and interdisciplinary teams
