Roles & Responsibilities
- Assess patients' mobility, strength, balance, and functional needs
- Develop tailored treatment plans that help patients regain independence
- Deliver hands-on therapy and instruct patients in therapeutic exercises
- Monitor progress, modify plans as needed, and document outcomes clearly
- Collaborate with nursing and interdisciplinary teams to optimize patient care
- Educate patients and families on safe mobility techniques and home programs
- Ensure compliance with facility policies and maintain accurate records
Qualifications
- Current California Physical Therapist license (required)
- BLS/CPR certification (or willingness to obtain upon hire)
- Experience in post-acute, skilled nursing, or outpatient settings preferred
- Strong clinical reasoning and excellent communication skills
- Ability to complete documentation in a timely, professional manner
Education
- Doctor of Physical Therapy (DPT) preferred; Master's in Physical Therapy accepted
Knowledge & Skills
- Gait training, therapeutic exercise, and balance interventions
- Use of mobility aids and adaptive equipment
- Skilled assessment and treatment planning for functional deficits
- Patient and family education techniques
- Proficient documentation and electronic medical record use
