Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long term care setting
• Administer medications and treatments accurately and document appropriately
• Monitor resident condition and communicate changes to the clinical team
• Support activities of daily living while promoting resident dignity and independence
• Collaborate with interdisciplinary team members to develop and follow individualized care plans
• Mentor and support CNAs and other staff during your shift to maintain high standards of care

Qualifications

• Current LPN license required (must be active and in good standing)
• Current Basic Life Support (BLS) certification required
• Comfortable working 12.5-hour night shifts and managing a full patient load
• Strong clinical judgment, excellent communication, and solid documentation skills
• Long term care experience preferred but new-to-LTC LPNs with strong skills are welcome

Education

• LPN certificate or diploma from an accredited program
• High school diploma or GED

Knowledge & Skills

• Medication administration and wound care knowledge
• Familiarity with electronic charting and accurate documentation
• Ability to assess, prioritize, and escalate resident needs
• Team-centered approach and strong interpersonal skills
• Basic infection control and safety practices
